EMPLOYEE CONSENT FORM ON THE INTEGRITY POLICY OF ACONE – ASSESSORIA E CONSULTORIA EMPRESARIAL LTDA

I understand that ACONE – Assessoria e Consultoria Empresarial Ltda. has an Integrity Policy and that, as an employee, it is my responsibility to be familiar with and follow the guidelines established by this policy. By agreeing to this consent form, I affirm that:

• I have read and understood the Integrity Policy of ACONE – Assessoria e Consultoria Empresarial Ltda.;
• I agree to follow all the guidelines and standards described in the Integrity Policy of ACONE – Assessoria e Consultoria Empresarial Ltda.;
• I commit to report any suspicion or incident of violation of the Integrity Policy of ACONE – Assessoria e Consultoria Empresarial Ltda. immediately to those responsible for the company’s compliance program;
• I understand that if I violate the Integrity Policy of ACONE – Assessoria e Consultoria Empresarial Ltda., I will be subject to disciplinary measures, including possible termination of my employment contract.
